Investigation reports are not provided electronically. The conference should be scheduled within 30 days of your appeal; you will meet with the DPH district manager and possibly the nursing home administrator. Within 10 working days after the informal conference, DPH should notify both you and the facility of its determinations. DPH must notify you of the name of the assigned investigator within two working days of receipt of your complaint. It is a good idea to call the assigned investigator prior to the onsite investigation to ensure the investigator is familiar with the details of your complaint and to discuss the planned investigation. If the evidence presented proves the misconduct, the Board may impose disciplinary actions which include suspension or revocation of a license, a reprimand, a fine, probation, or an order. Formal actions result in public orders that may be disclosed upon written request as required under the Maryland Public Information Act, and will be posted on the Board's website. When the investigation is complete, the Board may vote to dismiss the matter, or take informal action which may include letters of agreement, letters of education, and letters of admonishment. A letter of agreement may require the licensee to take corrective action.

If a resident, family member, representative or staff member is worried about a resident's care, they can file a complaint with the ombudsman's office. Residents of long term care facilities and their families have a right to good care. Nobody should suffer from abuse, neglect, discrimination, or retaliation. When all else fails, your best plan of action could be to file a formal written complaint with the Department of Health.
